盆腔炎附件炎的原因多与患者不良卫生习惯有关，如经期、产后一月内有性生活，妇科手术后一月内洗盆浴等。人工流产、引产消毒不严引起的医源性感染，也可引起盆腔积液。如有慢性感染病灶，可能是妇科系统如卵巢、输卵管盆腔内的炎症引起盆腔积液，也可能由结核或者肿瘤引起的，也有少数因宫外孕破裂、黄体破裂、盆腔脓肿、巧克力囊肿、卵巢癌引起。
